This notice is provided by Çerkezköy İmplant — Dentist Mehmet Uğur Alpay ("the clinic") under Article 10 of Turkish Law No. 6698 on the Protection of Personal Data (KVKK), to inform visitors to this website.

What we collect

Personal data is collected through this website only when you fill in the appointment or contact form:

• Full name
• Phone number and/or e-mail address
• Whatever you write in your message
• The date and time you submitted the form
• The version of the privacy notice you consented to

For security, the IP address of the connection is not stored in readable form — only an irreversible hash of it is kept. Your browser identification (user agent) and the page you arrived from are also kept as technical records.

Simply browsing the site without submitting the form collects no personal data.

Why we process it

• To assess your appointment request and get back to you
• To answer your questions
• To prevent abuse of the form (spam, automated submissions)

Your data is never used for marketing, advertising or profiling, and is never sold.

Legal basis

Appointment and contact requests are processed under KVKK Art. 5/2-(c), as directly related to the establishment or performance of a contract, and Art. 5/2-(f), the clinic's legitimate interest. The checkbox on the form also records your explicit consent.

Who we share it with

Form data is not transferred to third parties. It is stored on a server located in Türkiye. No data is transferred abroad.

Data is shared with public authorities only where a legal obligation arises.

How long we keep it

Form records are kept for at most 2 years and are then deleted automatically. If your appointment took place and treatment began, the retention periods required by health legislation apply separately.

Your rights

Under KVKK Art. 11 you have the right to: learn whether your personal data is processed; request information if it has been; learn the purpose of processing; know the third parties to whom it has been transferred at home or abroad; request correction if it is incomplete or inaccurate; request erasure or destruction; request that these actions be notified to third parties to whom the data was transferred; object to a result reached solely through automated analysis that works against you; and claim compensation for damage suffered.

You may submit requests in writing to the address above or in person at the clinic. Your request will be answered within 30 days at the latest.

Changes

Any change to this notice will be published on this page. The version of the notice you consented to through the form is recorded.
